Log into your Patient Portal to access your medical records.

What to Bring

Please bring the following items to your appointments:

• Your insurance card
• Photo ID
• List of all medications
• Co-pay, deductable or payment if uninsured

The Day of Your Appointment

Please arrive 15 minutes early for registration. We will ask you to complete forms and verify your insurance information is correct. We will also need to collect a co-pay or deductible before your appointment.

Please remember that your appointment time is valuable. If you are unable to keep your appointment please let us know at the earliest possible time so that we may offer your appointment spot to another patient. You will be charged a $25.00 no show fee if appointments are not canceled in advance.
